Avizent is pleased to provide a web-based claims reporting option for self-insured and insured clients. This is Avizent’s preferred method for reporting workers’ compensation claims; as well as auto, general and property liability claims.

This enhanced system offers clients the best of all options – it is quick, secure and absolutely free. It also provides increased accuracy and data integrity. Clients who use another method to report claims and would like to switch to our web-based service can do so immediately.

The Claim Intake form, located in the "File a Claim" section of our website, integrates directly with our claims management system, VISUAL Claims Studio™. Clients can access the Claim Intake form by simply clicking on the "File a Claim" button at the top of the website.

Claims can be filed by filling in the client, service, or policy number field. If needed, these numbers can be obtained quickly by contacting Avizent’s Customer Service team at 888-296-4283. After submitting the completed form, clients automatically receive an email notification with a claim number, along with a completed First Report of Injury (FROI). Our online system is available 24 hours a day / 365 days a year.

Our clients can also submit claims using a dedicated email address, fax number, or toll-free telephone number. However, these reporting options may include service fees in accordance with current client agreements.

Save Money, Report Claims Early

Claims should be reported to Avizent within 24 hours. However, claims that involve catastrophic accidents should be reported immediately. Employers can reduce the costs associated with a claim by reporting early. After a claim is received, a claim number is assigned and Avizent’s team works quickly to capture any outstanding information, verify and document coverage, and ensure compliance with best practices and jurisdictional requirements.
